This notice covers the information collected through this website. It is separate from the practice’s Notice of Privacy Practices, which covers your child’s medical record.
What this page covers
This website offers one form: a request to be contacted about an appointment. This notice explains what happens to the information you type into it. It does not replace the practice’s HIPAA Notice of Privacy Practices, which governs your child’s medical record and is provided to you as part of the new-patient packet.
What we collect
- Your name, phone number and email address.
- The office you prefer, your child’s age, and the reason for the visit, if you tell us.
- Anything you choose to write in the message box.
We ask you not to include detailed medical information in the form. It is a request to be contacted, not a secure channel for health information.
How it is used
This is a demonstration site. On the practice’s live site, a request would be emailed straight to the office so a staff member could call you back and arrange an appointment. On this demonstration build the form instead delivers to SiteFlip, the agency that produced the demo — nothing submitted here reaches Arizona Pediatrics, and nobody at the practice will see it or call you back.
Please use the phone number on the contact page to reach the practice. Requests sent through this demo are not used for marketing and are not sold or shared.
Analytics
This site uses Cloudflare Web Analytics to count page views. It is privacy-first: it sets no cookies, does not fingerprint visitors, and does not track individuals across sites or over time.
Security
The site is served over HTTPS, and form submissions are transmitted encrypted.
Children
This form is intended to be completed by a parent or legal guardian, not by a child.
