Welcome. Here is everything for a first visit.
Complete the packet before you arrive and registration takes a couple of minutes instead of half your appointment. Extra time is allotted for new patients.

Call or request a time
Tell us your child’s age and what the visit is for. If you want a Meet the Doctor visit first, say so — it is free.

Download and fill in the packet
English or Spanish. Doing it at your kitchen table beats doing it on a clipboard with a toddler on your hip.

Bring it with you
Along with your insurance or AHCCCS card, immunization records, and the name of your previous pediatrician if there was one.

Returning families updating an address or insurance only need the Patient Information Form — bring it on your next visit and we will gladly update your records.

What to bring
Your first visit
- The completed new-patient packet
- Your AHCCCS or insurance card
- Your child’s immunization record, if you have it
- The name and address of your previous pediatrician, for records
- Any medicines your child takes — the actual bottles are easiest
- Your questions. Extra time is allotted for new patients.
Office policies
How we handle calls, and what we ask of you
Calling during office hours
You are welcome to call us with questions about your child’s health during regular office hours. Dr. Saucedo and/or Dr. Antseliovich return phone calls between patients, at lunch, or at the end of the day. All messages are answered before the staff leaves the office.
If your message is urgent, please let the front desk know so we can respond immediately.
After hours
Phone calls received after office hours are returned by the physician on call — so you are speaking to a doctor who is familiar with your child’s health history.
If this is an emergency, call 911 or go to your nearest emergency department.
Cancellations
We appreciate 24 hours’ notice if your child’s appointment needs to be cancelled or rescheduled. That lets us give the slot to another sick child.
Extra time is allotted for new patients and for children with special health care needs.

Meet the Doctor — no charge, no obligation
Our doctors welcome parents to come in and meet them and the office staff — free of charge, with no obligation. It is a chance for your child to get comfortable with the office, and for expectant parents to have their questions answered before the baby arrives.
